The OMMP honors requests for a 30-day extension so long as the program receives a proper request before your OMMP card's expiration date.

How to request an extension for renewal due to personal hardship

1. State in writing that you are requesting an extension of your OMMP card’s expiration date.  You may want to make a copy of the request to keep with your card.
2. Include a brief reason why you are making the request (personal hardship, such as your appointment date is the only date that you could see the doctor)
3. Include date and signature
4. Make sure that OMMP gets the request before the expiration date
5. Mail request via certified mail or fax it and keep the mailing receipt or fax confirmation with the copy of the request and your OMMP card.
